Respiratory Physio2 ("we", "us") is committed to protecting your privacy. As a health service provider we handle your information in line with the Australian Privacy Principles (APPs) in the Privacy Act 1988 (Cth), and we aim to be open and clear about how we do it. Your health information is sensitive, and we treat it that way.

What we collect

We collect only the information we need to provide safe, effective care and to run our practice. This may include:

• Your name, date of birth and contact details.
• Your medical history, current medications, and relevant social and lifestyle information.
• Notes from your consultations, assessments and treatment.
• Your Medicare number and referral details (for GP Chronic Condition Management Plan claims), DVA or NDIS details where relevant.
• Private health fund membership details, and payment details for settling your account.

How we collect it

Wherever practical we collect information directly from you: over the phone, through our online booking system, on our registration forms and during your consultations. Sometimes we receive information from other people involved in your care, such as your referring GP or specialist, a hospital, or an imaging or pathology provider. We only obtain reports from third parties with your authorisation.

Why we collect it, and how we use it

We use your information to assess, treat and communicate with you about your care; to keep accurate clinical records; to process payments and rebates; and to send appointment reminders. We may also use de-identified information to help us review and improve our services.

Who we share it with

Your information stays within our practice unless sharing it is part of your care, you have asked us to, or the law requires it. In practice this means:

• Your treating team. We may share relevant information confidentially with your GP, respiratory specialist or other health providers involved in your care. If you are referred under a GP Chronic Condition Management Plan, we send your GP a letter after your first visit and at the end of your plan. Let us know if you would prefer we didn't.
• Medicare, DVA, NDIS and health funds, to the extent needed to process claims and rebates.
• Legal requirements, such as mandatory public health reporting or a court order.

We do not sell your information, and we do not share it for marketing.

How we store and protect it

Our clinical records are stored electronically in Cliniko, a secure practice management system that encrypts data and is hosted in Australia. Access is limited to authorised staff with individual, password-protected logins. Any paper records are kept securely and destroyed or de-identified when no longer needed. All staff are trained in their privacy obligations.

We keep health records for the period required by law (generally at least seven years from your last visit, or until a child turns 25), after which they are securely destroyed.

This website and online booking

Bookings made through this site are processed by Cliniko under its own privacy policy. Our website does not use advertising or tracking cookies. The map on our contact page is provided by Google Maps, which may set its own cookies when it loads.

Accessing or correcting your information

You are welcome to ask for a copy of the information we hold about you, or to have it corrected if it is inaccurate or out of date. Contact us using the details below and we will respond promptly. A small fee may apply to cover the cost of copying extensive records.

Questions or complaints

If you have a question or concern about how we have handled your information, please talk to us first. We take complaints seriously and will do our best to resolve them quickly. If you are not satisfied with our response, you can contact the Office of the Australian Information Commissioner (OAIC) at www.oaic.gov.au, by phone on 1300 363 992, or by post to GPO Box 5218, Sydney NSW 2001.
